We assess Home (place of dwelling) Environment for risks and barriers to promote safe living. We may assist you in:

• Determining how to keep your home environment safe for you and your family
• Making your home safe and barrier free.
• Assessing risks and barriers

• Mobility and Seating Assessment to select appropriate mobility device such as wheelchair, walker, power wheelchair or scooter.
• Feeding Assessment
• Behaviour and Cognitive Assessment
• Bathroom Safety Assessment
• Environmental Safety Assessment
• Falls prevention

We coordinate safe and timely discharge of our clients by performing the following:

• Acting as a liasion between with rehabilitation team, community resources, hospital staff, and funding sources and legal representative
• Conducting appropriate assessments such as Attendant Care and Home Safety Assessments
• Coordinating order and delivery of assistive devices in coordination with the rehabilitation team
• Arranging for community services such as Personal Support Worker (PSW) Services, Nursing, Physiotherapy (PT), Occupational Therapy (OT), Social Work (SW) and Speech Language Pathology (SLP)

If you have been away from work due to injury or illness, we may assist you in:

• Preparing for return to work with cooperation and partnership with your employer
• Reconditioning your skills to return to work
• Creating and implementing a gradual return to work plan
• Providing support, guidance and coaching
