Attendant Care FORM1 Assessment

We assess our clients` current abilities to perform their pre-injury Personal Care Tasks such as bathing, dressing/undressing, feeding, and toileting.

Functional Ability Evaluation (FCE/FAE)

We assess our clients` Functional Abilities.

Cognitive Assessment

We assess our clients` cognition or cognitive level of functioning to assesses whether they have cognitive impairment or not. Our experienced therapists provide treatment, consultation and recommendations to improve cognition to enhance quality of life and promote independence.

Job Site Analysis (JSA) and/or Physical Demands Analysis)

We assess our clients` pre-injury or current job duties and physical demands at their place of employment.

Home Safety Assessment

We assess Home (place of dwelling) Environment for risks and barriers to promote safe living. We may assist you in:

Home Modification Assessment

We assess Home (place of dwelling) Environment for architectural and environmental barriers to enhance independence & safety.

Discharge Planning

We coordinate safe and timely hospital discharge by:

Acting as a liasion between clients and the rehabilitation team, community resources, hospital staff, and funding sources and legal representatives.
Conducting appropriate assessments such as Attendant Care and Home Safety Assessments
Coordinating order and delivery of assistive devices in coordination with the rehabilitation team
Arranging for community services such as Personal Support Worker (PSW) Services, Nursing, Physiotherapy (PT), Occupational Therapy (OT), Social Work (SW) and Speech Language Pathology (SLP)
Coordinating and completing insurance related documentation

Return to Work and Work Conditioning:

If you have been away from work due to injury or illness, we may assist you in:
